1. The Application

•All applications must be completed using CAPITAL LETTERS in the applicants own handwriting using a black or blue ballpoint pen
•An applicant who is not able to complete the form for various reasons such as disability, illiterate, non-English understanding, may have the form completed by a third party who must make a statutory declaration, stating that he/she has correctly completed the application form with details supplied to him/her by the applicant
•The declaration should be submitted with the application
•Applications under the age of 16 should have their parents’ consent with copies of parent’s passports and birth certificates
•Applicants must provide 2 passport photos
•Applicant should NOT be wearing any form of head gear
•The reverse side of the photograph is to be witnessed and certified by a third party as follows: ‘I certify that this photograph is a true likeliness of ‘--------------------‘(name of applicant) and must be signed by the witnessing officer.
•ALL applicants must provide a left thumb print and signature in the boxes provided.
•If a person is illiterate, he/she should provide a left thumb print at item 15 or 20 in the two boxes provided. The print must clearly show the distinguishing lines of the thumb.

2. Passport Requirements

An applicant who is in possession of a passport must first surrender the passport when applying for a replacement. Any application for a passport for the first time should be accompanied by the applicant’s birth certificate (Full Extract) or documentary proof of Citizenship and 3 photographs (see top of form). For a married woman, attach your marriage certificate. Adoption Papers for adopted children. Deed Poll Paper for change of name. Decree Nisi and Decree Absolute for Divorcees. School Reports for Students. In addition provide any of the following as additional identity; (I) FNPF Membership Card (ii) Drivers License (iii) Bank/Business Card.
Under special circumstances, current passports can be retained for work related purposes however, applications for renewal of passports should include; full colored copy of the current passport with the particulars page; colored copy of the UAE work permit, 2 passport size photos with the original Application for Passport, passport fees and a letter indicating why the original passport cannot be returned to Fiji for renewal purposes.

a. Requirements for First time applications

• 2 passport size photos, exactly 50mm by 40mm in size
• Original full extract of the birth certificate
• Original full extract marriage certificate if applicant is a married woman and wishes to use her husband’s surname
• If divorced then original certified copies of Decree Absolute
• If spouse is deceased then original death certificate to be provided
• Copy of deed of poll if applicant has changed his/her name
• Adoption papers if applicant is adopted
• Another form of ID, driving license, FNPF, water bill, Bonafide letter from school
• Student applicant to provide school report, or school identification card
• Payment of requisite fees.

b. Renewal of Passport

Fill in the application form with:
• 2 recent passport size photographs
• The current or expired passport
In case if the current passport is lost, provide the following:
1. Original full extract of the birth certificate
2. Original full extract of the marriage certificate
3. Original full extract of the marriage certificate-if applicant is a married woman, and wishes to use her husband’s surname
4. If divorced, then original certified copies of Decree Absolute
5. If spouse is deceased, then the original death certificate to be provided
6. Copy of Deed of Poll if the applicant has changed his/her name
7. Adoption papers if applicant is adopted
• Another form of ID, driving license, FNPF, water bills, bonafide letter from school
• Payment of requisite fees

c. Replacement of Lost / Mutilated Valid Passport

Submit in a completed application form with:
• 2 passport size photographs
• Original full extract of the birth certificate
• Original full extract of the marriage certificate
• Original full extract of the marriage certificate, if applicant is a married woman and wishes to use her husband’s surname
• If divorced, then provide original certified copies of Decree Absolute
• If spouse is deceased, then provide original death certificate
• Provide a copy of Deed of Poll if applicant has changed his/her name
• Adoption papers if applicant is adopted
• Another form of ID, driving license, FNPF, water bills, Bonafide letter from school
• Student applicant are to provide school report, or school identification card
• Police report on how the passport was lost
• Statutory declaration stating how the passport got lost
• Payment of requisite fees

d. Replacement of Damaged Passport

Fill in the passport application form with the items listed below:
• 2 passport size photographs
• The damaged passport or documents to be submitted if the passport details have been mutilated
• A statutory declaration stating how the passport was damaged
• Payment of requisite fees

3. Signature

Signature should be in the applicant’s own handwriting inside the allocated block. For an illiterate, the left thumbprint should replace the signature, but the print must clearly show the distinguishing lines of the thumb. Smudged or blurred prints are not acceptable.

4. Entitlement to a Fiji Passport

Any Fiji Citizen is entitled to a Passport irrespective of age provided all normal requirements are met.

3. Commissioner of Oaths
4. Magistrate
5. Employer
6. Senior Police Officer
7. Minister of Religion
8. Registered Consultant
9. Businessman
10. Senior Civil Servants; or
11. Senior Post Holders in any Organization
*Note: Authorization will only be accepted of the personnel listed above if authorization is done by someone apart from the listed personnel than the document will not be accepted or taken into consideration. 

6. Passport Fees

The current scale of fees (VAT inclusive) for passports is shown below and payment should be made payable to the Embassy of the Republic of Fiji in the United Arab Emirates in cash, international money order or international bank draft for the exact amount:
Application for Renewal of Passport F$76.00
Application for a Urgent Passport F$190.00
Application for a Replacement of a lost/mutilated valid passport F$304.00
Application for a Certificate of Identity F$140.00
Endorsement to a passport after it has been issued F$76.00

Additionally, applicants should pay F$40.00 as courier fees (DHL) for applications for passports.

7. Place of Issue

Passports are issued by the Immigration Department Headquarters, Suva, Fiji during office hours. Completed application form should reach the Immigration Department not less than 14 days before the date of which the passport is required.

8. Passport Validity

A passport is valid for 10 years from the date of issue.
